Should this menu auto refresh, or is there a way to set it to auto refresh every X seconds? I was on my site earlier (legendsguild.dkpsystem.com)and another member was on, but the list doesn't auto refresh, so unless he or I went to another page or refreshed manually, one of us may not see the other online.

Loading a site is what triggers the site to know who's online. It functions like most "Members currently online" parts of forums, and to lighten the load, it doesn't auto-refresh without accessing the site by loading a page.
